When individuals cannot maintain a home of their own because of a temporary or chronic physical condition or mental disability and more care is needed than can be provided by family, in home aid services, or community services, care must be considered outside of the home, Adult Care Home placement is sometimes the answer to meeting their needs. These homes are often referred to as rest home or assisted living homes. they provide residents 18 years or older with supervision and personal care as needed, assistance with planned activities, and transportation to community activities.

There are times when people can no longer perform the tasks of daily living even with the help of family, in home aides, and community services. Care outside the home must be considered. their needs can not be met in an Adult care Home. they do not need to be in a hospital, as those beds are needed for patients with acute illnesses. The care needed can be received in a Nursing Home. Nursing Homes provide nursing services by, or under the supervision of, licensed nurses according to physicians orders.

Adult Care case management Services is offered to an individual living in an adult care home who is in need of heavy care. Heavy care patients require extensive assistance or are totally dependant on others. The resident must be a Medicaid recipient. The case manager insures that the residents health care needs are being met.

| Adult Protective Services |

Protected Services for Adults is responsible for assuring that disabled adults are protected from abuse, neglect, and exploitation. Those who may be in need include persons with disabilities which keep them from caring for themselves.
A social worker is available even after regular working hours to intervene and assure that adults who are at risk are protected.

| Hospital Social Work Services |

The Hospital Social Worker is a source of information and referral. He/She has available a list of community resources. Referrals are made based upon need. The worker acts as a liaison between the Hospital and the Department of Social Services. Social Work Services are available to all hospital patients. Services or a visit by the social worker may be obtained by the patient's request or a request from a physician or nurse.

Social worker for the Blind provides services to the blind and visually impaired citizens of Wayne County, Including children and adults, to enable them to become independent, provide a constructive contribution to society and realize their potential.
Services such as instruction in independent living as well as specialized equipment are available.

| Eye Care Certification |

Eye care consists of eye examinations, eyeglasses, eye treatment, eye surgery, hospitalization for eye disorders, ocular prostheses, and eye medications. A person may be certified as eligible for an eye exam and/or eye glasses every six months. The eligible person must pay $5.00 towards the total costs of the examination and $5.00 for the glasses.
Eye exam certifications are done on Tuesday from 9:00 am until 12:00 pm and on Wednesdays from 1:00 pm to 4:00 pm at the Department of Social Services Annex. Customers need to provide the family’s income, and any outstanding medical bills that are in the process of being paid.
