Many patients and/or families face difficulties during a time of illness and recuperation. The Social Worker's role in the medical setting is to assist patients and their families in obtaining any services that will maximize the quality of care.
The Social Worker is available to discuss any problems or concerns that may be causing the patient undue anxiety. Referrals for further services are available upon discharge.
Assistance in arrangement for in-home services, durable medical equipment, and referral for placement in an extended care facility is also offered.
The Hospital Social Worker is a source of information and referral. He/She has available a list of community resources. Referrals are also made to out-of-county agencies based upon the individual's needs. The Social Worker acts as liaison between the hospital and the Department of Social Services. He/She is able to interface and act upon various services offered by both agencies.
Social Work Services are available to all hospital patients. Professionalism and the patient's right to privacy are maintained in all situations. Services or a visit by the social worker can be obtained by the patient's request or a request from a physician or nurse.
