A Healthy Child is a Smart Child
There are two insurance programs in North Carolina available for children who qualify. To be eligible, your family income must be less than that listed on the table to the right. Families who earn more than the listed amount may still qualify if they have day care or work related expenses, or are making court ordered child support payments.
Many children will be eligible for Health check. Children can qualify for this program up to age 21. Children, up to age 19, who do not qualify for Health Check may qualify for NC Health Choice for Children. Families only need to fill out ONE application form for all children living in the home, to apply for either program.
Family Size Monthly Income* 2 $2,282 3 $2,862 4 $3,442 5 $4,022 6 add $580 for additional family member * Income before taxes. Children of families who earn more may qualify if they have daycare, work-related or child support expenses. This chart is valid through March 31 2008
Health Check/Health Choice insurance covers the following:
Checkups at a doctors office. Health care for your child when he is sick. Checkups and treatment at the dentist. All the shots your child needs to start school or day care. Treatment for health problems that could affect your child's life or how he learns at school. Tests to catch any health problems early before they become worse.
Health Check/Health Choice can pay for all or part of your child's health care, including:
Well-Child Checkups Medicines Sick Visits Denatl Care Immunizations (shots) Hospital Care  Lab Tests Counseling Therapies Surgery Medical Equipment & Supplies  Vision & Hearing Care Additional benefits may be available for children with special health care needs.
