Information and Assistance
Information and assistance is provided to older adults, their families, and others in an effort to acquire information about resources available to meet their needs.
The goal is to match resources to individual needs. The needs may be broad or specific, and the coordinator works to provide necessary referrals for each occurrence. Some requests include:
- In-Home Aide Program
- Private Sitters
- Legal Services
- Respite Program
- Emergency Assistance
- Financial, heat, food, medical
- Durable Medical Equipment (DME)
- Wheelchair ramps, wheelchairs, etc.
- Advocacy, Abuse, Neglect services
- Insurance Counseling
- Home Repairs, and housing renovations
- Long-Term Care assistance
- Housing Assistance
- Nutrition Assistance
- Agency Referrals
- Transportation
